Providence in Horizon Adult Family Home provides individualized, resident-centered care in a safe and comfortable home environment. We assist with activities of daily living, including bathing, dressing, grooming, mobility support, and toileting, while promoting independence and dignity. With only six residents, we are able to offer attentive, personalized care tailored to each individual’s needs.
RN-Managed Health & Medication Support
Our home is owned and managed by registered nurses, ensuring professional oversight of health monitoring and medication management. We provide safe medication administration, routine health assessments, coordination with primary care providers, and support with chronic condition management. Our RN-led approach helps families feel confident knowing their loved ones receive skilled, compassionate care.
Daily Activities, Nutrition & Quality Living
We believe quality of life is just as important as quality care. Residents enjoy meaningful daily activities designed to encourage social engagement, mental stimulation, and physical well-being. We provide nutritious, home-cooked meals in a warm dining environment and maintain a clean, peaceful home in a quiet neighborhood where residents feel comfortable, engaged, and at home
